Tournament Registration

Please complete all sections.

Your team's registration will be confirmed once the following steps are completed:

1. Payment $800.00 paid via etransfer [email protected]
or via  cheque  at:

Coldwater Minor Hockey Association

Coldwater, Ontario
L0K 1E0
        Indicate team name and division on the cheque.

        *In the event a payment is returned NSF, an additional charge of $50 will be
          applied/due.

 


2. A copy of the OMHA-approved team roster is provided by email to [email protected]
*$500 team deposit guarantees acceptance to tournament provided availability. 
   All fees are due 60 days prior to  tournament
