Registration 2022/23 Season (Coldwater Minor Hockey Association)

1. Players must accept any team and/or league changes as seen fit by the Executive. Refusal can result in dismissal without a refund.
2. I will not hold the Association, its officers or members responsible for any injuries incurred by my child/children while travelling or playing for the Coldwater Minor Hockey Association.
3. Insurance is the responsibility of the parent or guardian.
4. I understand that I am required to provide to the Association proof of my player's date of birth. Failure to do this can result in my player(s) not being allowed to play until such documents are forthcoming.
5. "Proof of Age" documentation is determined by the OMHA and this Association.
6. Privacy of your personal information is an important part of the Coldwater Minor Hockey Association commitment to parents and volunteers during and after your participation in the organization. We understand the importance of protecting your personal information. The Personal Information Protection and Electronic Documents Act (PIPEDA) took effect in January 2004. PIPEDA seeks to balance an individual's right to privacy with the needs of our association to collect, use or disclose personal information for legitimate business/association purposes. PIPEDA indicates that no personal information (which includes application information and criminal background information) identifying an individual is to be disclosed without the specific consent of the individual. By agreeing or volunteering with our association, you acknowledge your awareness of this legislation and understand our purpose for collecting the information.
